Reports are used for information of a crime and not necessarily meant for someone committing a crime. Examples will be listed below.
Incident Report
Used to record statements of a incident that involve witnesses.

Officer Involved Shooting Report (OIS)
Used for supervisors to record shootings involving officers of an agency.

Motor Vehicle Accident Report
Records all motor vehicle involved accidents.

A supervisor will sometimes contact you to fix some grammar errors, or lack of mentioning the probable cause of the crime committed.
